The correct name is Shu Han.
Shu Han is called so because it was the only power that still went (and tried to revive) the Han Dynasty.

This is sorta the wrong place for this, but since it doesn't do any harm it doesn't really matter.
We know all about how it was called the Shu-Han, and in reality they probably even preffered to be called the han instead of just Shu.
But since a lot of people never actually looked at the three kingdoms but only dynasty warriors they don't know that, so its a way of just keeping things simple so everyone understands
