No there are nearly no changes other than they added 7 islands after you beat the 7th gym.
The johto remakes are much better.

[Pokemon History for newbs]
Pokemon Green came before Pokemon Red and Blue; it was glitchy and the graphics are even more retro. I think it only came out in Japan and some parts of America. Traditionally considered the first Pokemon game. Blue and Red were then released which were basically remakes of Green with very little difference other then update of sprites and fixtures of plotline and glitches. Then Yellow came which was a more sell-out game which allowed the player to play a more fitting story like the Episodes (You could catch all the starters like Ash did in the anime.)
Then Pokémon turned away from Kanto, the first region before expanding to Johto, inviting the newer pokemon. This is my first and favourite pokemon section. I missed out on the first lot for a few years due to me being one of the younger pokemon players. I eventually picked up Red and Yellow but it didn't compare to Silver. Silver and Gold eventually followed by Crystal contained both Kanto and Johto, making a total of 16 Gyms; beating all the others which have only ever contained 8.
Once more after the traditional three games. Nintendo moved onto a new region. Hoenn. Ruby/Saphire and Emerald. They now moved onto the Gameboy Advanced opening trading capabilities a little more and adding tonne more pokemon and mini-games. The sprites were completely revamped and things improved. This is my second favourite, they added a tonne of things that made the whole system interesting.
Leaf Green and Fire Red came next, remakes of Red and Green/Blue. Basically upgraded versions of the old games with newer sprites and the ability to catch the newer pokemon. They are alot of fun; a refreshing outlook upon ones childhood and definately worth a purchase. Still on the GBA.
Now with the new handheld console DS on the market Nintendo; Nintendo turned to its newest and latest region. Sinnoh. This is deemed the place where pokemon as a franchise died, with calculating to over 500 pokemon and many of them being daft. Older generation players have complained about this game. Having played Pearl, Diamond and Platinum. I personally didn't mind it. It isn't as good as the old ones; but theres alot of cool stuff to do and the pokemon aren't that bad. The battle system has improved alot!
Finally the latest released pokemon games for the handheld would be SoulSilver and HeartGold. The remakes of Silver and Gold. My favourites. I picked this up on the day it came out. Being an avid fan of the pokemon franchise since a kid. I loved them, they stay true to the plot yet add enough fun. Even from the smallest detail as the pokemon following you around now or Kurt making his pokeballs in the hut. I think this is what die hard pokemon fans have been waiting for.. however it died on my early. It gets boring once you've beat all the gyms and it appeared alot more easier to me then the older games. Completed it in a matter of a day or two.
Finally, pokemon have announced Black and White versions. Which will be a new region with new pokemon :/ (Great even more!) However they have yet to announce remakes of Sapphire and Ruby which is clearly what we are want!
My Recommended Games to Get:
Either LeafGreen/FireRed Either Ruby/Sapphire or Emerald Either SoulSilver or HeartGold
If your feeling brave...
Pokemon Pearl/Diamond or Platinum
